Job Description
Full-Time, Regular grant-funded position at the DWC. This is under the Bristol Meyers Grant that works on low-dose CTs for cancer prevention | Schedule: Mon-Fri 8a-4:30p | Pay: Paygrade 13, Hourly, Dependent upon experience

Job Purpose or Objective(s): The Case Manager RN provides nursing navigation support for individuals participating in the lung screening program and promotes continuity of care throughout the screening process within the Diabetes Wellness Center Lung Cancer Screening Services.
Primary Tasks
- Assess patient needs and develop case management plans to support individuals participating in the lung cancer screening program.
- Review patient information with the interdisciplinary care team to plan, modify, and support screening, diagnostic, and follow-up services.
- Educate patients on screening procedures, treatment options, and next steps to support understanding and informed participation in care.
- Maintain case records and complete required reports for CNHSA and applicable screening program requirements.
- Manage patient cases to ensure timely follow-up care, diagnostic evaluation, and treatment services related to lung screening.
- Schedule consultations, referrals, and diagnostic tests to support timely access to screening and follow-up care.
- Coordinate communication among patients, providers, and care teams to support continuity of care throughout the screening process.
- Develop and recommend workflow improvements that support effective lung screening services across CNHSA hospitals and outpatient clinics.
- Review referrals, confirm eligibility, and enroll qualified patients in the preventive lung cancer screening program.
- Perform other duties as may be assigned.
Job Requirements
- Associate degree in nursing
- Current RN licensure with the Oklahoma State Board of Nursing or Multi-State RN License
- Current BLS certification
- One [1] year Navigation experience or;
- One [1] year of case management experience
