Job Description

Regular, Full-Time position under the Aboha grant that aids in assisting tribal members across the reservation who are at risk of, or experiencing, homelessness | Schedule: Monday – Friday 8am-4:30pm | Pay: Salaried, starting between $58,000 – $60,000 per year

Job Purpose or Objective(s): As the Case Manager in Health Services (HS), you will plan, organize, and implement education and prevention material to communities, schools, and other agencies within the service area in order to create awareness and provide guidance and support to victims. You will report to Behavioral Health Management. 

Primary Tasks

  • You will plan training following established and evidence-based practices.
  • Manage cases for victims following the problems and needs of each individual patient to include; assistance locating housing and medical care, education assistance, higher level of psychiatric treatment, transportation resources, link patients with necessary community resources and other needs that may arise.
  • Create awareness of assistance programs within communities through networking, community-based activities, and other scheduled activities to sustain community interest and participation.
  • Coordinate trainings to educate providers and community members about different trauma, violence and drug topics.
  • You will conduct a Coping Skills Group focused on anger management, healthy relationships, and other issues.
  • Collect data on services and numbers of people participating in each program.
  • Perform other responsibilities as may be assigned.

Job Requirements

  • Bachelor’s degree or higher in Psychology, Social Work, Counseling or related human services or equivalent combination of education and experience
  • Intermediate general computer and MS Word processing software skills
  • Good customer service skills
  • Understand the process of crisis stabilization and crisis response
  • Excellent communication skills and the ability to work as a team member
  • One [1] year of experience in a behavioral health field providing rehabilitation services to children, adolescents, and adults
