Case Manager (Tribal Opioid Response)

Description

Job Purpose or Objective(s): Plan, organize, and implement education and prevention material to communities, schools, and other agencies within the service area in order to create awareness and provide guidance and support to victims.

Primary Tasks:

  1. Plan education and training in accordance with established and evidence based practices.

  2. Manage cases for victims in accordance with the problems and needs of each individual patient to include, but not limited to; assistance locating housing and medical care, education assistance, higher level of psychiatric treatment, transportation resources, link patients with necessary community resources and other needs that may arise.

  3. Create awareness of assistance programs within communities through networking, community based activities, and other regularly scheduled activities to sustain community interest and participation.

  4. Coordinate trainings to educate providers and community members about various trauma, violence and drug topics.

  5. Conduct a Coping Skills Group focused on anger management, healthy relationships, and other issues.

  6. Collect data on services and numbers of people participating in each program.

  7. Perform other duties as may be assigned.

Required Education, Skills and Experience:

MINIMUM

  • Bachelor’s degree or higher in Psychology, Social Work, Counseling or related human services or equilent experience.

  • Intermediate general computer and MS Word processing software skills

  • Good customer service skills

  • Understand the process of crisis stabilization and crisis response

  • Excellent communication skills and the ability to work as a team member

  • 1 year of experience in a behavioral health field providing rehabilitation services to children, adolescents, and adults

TARGET

  • Master’s degree or higher in Psychology, Social Work, Counseling or related human services

  • Intermediate general computer and MS Word processing software skills

  • Good customer service skills

  • Excellent communication skills and the ability to work as a team member

  • 3 years of experience in a behavioral health field providing rehabilitation services to children, adolescents, and adults
