Job Summary: Serves as the Medical Coder for Choctaw Nation Health Care. Enter information utilizing the RPMS and EHR system for each visit. Define and code the purpose of visit. Perform data management. Performs correction through the Coding Queue System. Performs other duties as needed or assigned.

Primary Tasks:

  1. Enters information utilizing the RPMS and EHR system for each visit. Demonstrates accuracy and performs daily with no backlog (10 days) (5 days for Ambulatory). Maintain no more than 2 errors per 50 records.

  2. Define and code the purpose of visit using the ICD-10-CM, CPT and 3M coding software.

  3. Meets minimum standard, 125 encounters and 1250 dependent count daily, (“by 90 days”) (156 encounters and 1560 dependent count for 10 hour days) [ER coding 75 and 750, or 100 and 1000 if ER’s mixed with other coding (Hospital)] 90% of the time.

  4. Generates Visit Review Coding Queue, which involves a thorough review of a variety of error reports weekly, which requires pulling charts and reviewing the documentation resulting in deleting or appending data to an incomplete visit.

  5. Display multiple visits on the same day for a patient merging the visits if appropriate, applying the appropriate ICD-10-CM Diagnosis/Procedure; multiple primary providers and responding to a wide variety of other edit errors.

  6. Assist in training for healthcare providers and health record personnel in concepts, procedures, features and usage.

  7. Maintain knowledge of official coding rules/regulations and DRG assignment. Maintain knowledge of the Office of Inspector General Compliance Standards. Demonstrate a thorough knowledge of applicable transaction codes, ICD-10, CPT and E&M level of coding guidelines.

  8. Ambulatory Surgery: Accurately abstracts and code ambulatory surgery charts using ICD-10-CM and CPT. Performs chart reviews as needed. Send deficiencies to providers to complete as needed. Enter charges into the RPMS system from charge tickets and verify information from the medical record. Meets the minimum standard of 32 encounters per day for Ambulatory coding. No more than 5 days out.

  9. Other duties as may be assigned with scope and competency of the associate.

Required Education, Skills and Experience:

MINIMUM

  • High School Diploma or GED; or a combination of equivalent education and experience.

  • At Least 1-3 years’ experience with coding. Working knowledge of ICD-10-CM including Volume 3 (procedure coding) and have completed Medical Terminology, Human Anatomy and Physiology and Medical coding classes.

  • Credentials required: CCS, CIC, CCA, CPC with experience, COC with experience, CCS-P with experience.

  • Must pass basic ICD-10 coding exam with 75% or higher.

  • Demonstrates Good typing skills with accuracy.

  • Efficient time-management-able to work independently.

  • Demonstrates excellent organization skills, organized, motivated and detail oriented.

TARGET

  • Associate or Bachelor’s Degree in Health care related field.

  • At least 3 year experience with coding. ICD-10-CM coding including Volume 3 (procedure coding) and have completed Medical Terminology, Human Anatomy and Physiology and Medical coding classes.

  • Credentials required: CCS, CIC or RHIA/RHIT with experience

  • Must pass basic ICD-10 coding exam with 90% or higher.

  • Demonstrates Good typing skills with accuracy.

  • Efficient time-management-able to work independently.

  • Demonstrates excellent organization skills, organized, motivated and detail oriented
