Don’t worry, we have you and your family covered!
Choctaw Nation has a state of the art Wellness Center that is FREE to you and your dependents.
The center is open 24 hours a day 6 days a week, to accommodate all work schedules; there is also a variety of classes offered at no
additional charge. Not only is Choctaw Nation committed to helping you and your family reach your fitness goals, Chief Gregory
E. Pyle and Assistant Chief Gary Batton are proud to announce that the Choctaw Nation Wellness Centers in Durant and Hugo are open
free of charge to all active duty military, reservist and National Guard members and their dependents. Service members do not have to be
Native American to take advantage of this offer.
As an associate at Choctaw Nation you will quickly find that Health and Wellness is important to us,
and that is why we encourage all associates to participate in the “Thrive Program”. The THRIVE Program is
completely voluntary and is designed to encourage and support healthy behaviors. Some of the areas available

Choctaw Nation’s employee health clinic is
conveniently located at 622 Bryan Drive in Durant, OK.
The clinic is available to all associates of the Choctaw Nation at no charge.
The clinic is designed to meet your urgent care needs; you may use the clinic regardless
of CDIB status or health insurance status. The clinic is not intended to replace your primary care phycisian,
but available for your convenience. Walk-Ins are welcome and care is confidential.

The Choctaw people believed in education as a way to promote each other, learning from one another and from others
they encountered. Education has always been an important aspect of the Choctaw culture and continues in our traditions.
Building better foundations for associates for a better Choctaw Nation. This policy establishes guidelines for the Employee
Education Assistance Program (EEAP). The EEAP provides associates with financial assistance to attend an accredited college or
university for the purpose of pursuing an associate, bachelor, master, or doctorate degree that may prepare the associate for career advancement
within the Choctaw Nation of Oklahoma. The Employee Education Assistance Program (EEAP) shows commitment and support to the
educational initiatives and professional development of associates of the Choctaw Nation of Oklahoma. Under current tax rules, the program reimburses
up to $5,000.00 per calendar year for eligible education expenses.
Choctaw Nation of Oklahoma sponsors a 401(k) plan to assist you in saving for your retirement. Full-time and part-time associates
are eligible to participate upon attaining age 21 and completing 1 year of service. When you are eligible to participate in the plan,
you will automatically be enrolled, unless you elect otherwise. Under Automatic Enrollment, 3% of your paycheck before taxes will be
contributed unless you elect not to make Elective Deferral Contributions or to increase or decrease your Elective Deferral Contribution percentage.
Choctaw Nation of Oklahoma will make a Matching Contribution up to 5% based on your individual contribution.

Just when you think you have life all figured out, along comes another challenge. Whether the Challenge is big or small, your Life Assistance &
Work/Life Support Program is available to help you and your family find a solution and restore your peace of mind. All associates will have access
to the website and/or telephone number so they are able to contact someone for assistance any time of the day or night. Get extra support for handling
life’s demands, call for advice or a referral to a service in your community. This benefit is free 7 days a week 24 hours a day, call and get the assistance
you need to help resolve life’s challenges.
Not only do we offer Life Assistance sources we also have legal sources available as well. below are some of the following resources and legal documents you are able to have right at your fingertips:
Educuational information about estate planning issues, receive access to a variety of legal documents such as Durable Power of Attorney, Living Will, Standard Will, Medical Treatment
authorization and Identity Theft. Customize your plans for both you and your spouse, use the guidebooks, planning tools and more to help you prepare for you or a loved one’s final arrangements.
Get organized and create a valuable record of information for your family with the Personal Information Organizer, protect your identity with an Identity Theft Kit, including materials on prevention
and victim resolution actions. This benefit is also FREE to all associates of the Choctaw Nation.
To honor those associates that have chosen to honer their country the Choctaw Nation offers an additional
240 hours paid Military Leave for any scheduled workdays that are missed to participate in military obligations.
If for any reason the service member is called to active duty their job is protected while they are away. They
will also be offered supplemental income if their military pay is not as much while they were receiving their regular paycheck.
